Our practice is committed to providing you and your family with safe, gentle, and high-quality dental care. We understand that you or your child may feel anxious about visiting the dentist. We are sensitive to your needs, and it is our goal to make you feel comfortable visiting our practice while we provide the best possible care.
Once you have been scheduled for an appointment with our office, you will receive our new patient forms electronically to fill out in advance. This saves time and allows us to treat you promptly. If you would prefer to print out and complete the patient forms on paper, please see the links at the bottom of this page.
Please bring the following to your first appointment:
- New patient forms (electronic or paper)
- Drivers license
- Insurance card
- List of medications (if applicable)
During your first visit, we will review your medical history, take X-rays of your teeth and mouth, and perform a thorough exam. Then Dr. Thatch or Dr. Wire will meet with you to discuss their findings, help you create your individual treatment plan, and answer any questions you may have. We look forward to meeting you!
